To enroll, please print and complete these forms. Or, set up a visit and we will have a pre-printed enrollment packet for you.

When completed, please bring it in or email it to teacher@rhymereasonschool.com

*Upon completion, there is a $50.00 non-refundable application fee and half of one month’s tuition cost.
